1095-C Information
Due to recent changes in legislation, including the Paperwork Burden Reduction Act, NSD will not be posting individual 1095-C forms. If you would like a 1095-C form, you can request one, and we will provide it to you within 30 days of receiving your request. However, per the IRS website, this form is not required to complete your tax return, and you do not need it to file your taxes. To request your 1095-C either email humanresources@nsd131.org or mail your request to Nampa School District, 619 S Canyon Street, Nampa, ID 83651 (Attention: Benefits).
